Topic- Medication History

Therap's medication history form is the form used to create the electronic MAR. Sponsor's are required to enter the data in the med history form as well as uploading scripts or orders for medication verification. Sponsor's are responsible for the medical care and usually have the information about new medications, changed medications or discontinued medications before anyone else does. Assistance and supports should be provided by the QIDP, or the Medical Quality Assurance Coordinator as needed.

Topic- Health Tracking Forms

Therap's Health Tracking forms are used to chart that we are monitoring critical medical conditions. Medical conditions such as Diabetes, Seizure Disorders, Bowel Disorders, and Vitals are monitored daily. Diabetic checks, and vitals should be charted after each check is completed. Other health tracking forms should be charted by the end of the day for sponsors at a minimum. Other health tracking forms include, weight, menses, immunization, and skin would for injuries.

Topic- Controlled Medication Counts

Controlled medications must be once in a 24 hour period of time and when passed.  Controlled medication must also be verified. Meaning that the count must be validated as correct. The witness to the counts does not have to be med certified, but must be a W-2 employee or contractor. When a sponsor has controlled medications they are assisting with and reside alone, the counts should be verified by relief staff when available or the sponsor QIDP during quarterly qa checks. 

Controlled medication counts will now be completed on the eMAR.

Medical appointments are a critical aspect of medical documentation. Appointments should be entered at the time they are scheduled and protocols for rescheduling should be maintained. Medical appointments are part of the individual's permanent medical record. They should provide a detailed medical history and account of all medical interactions.
