Sponsored Residential Training
Sponsored residential services are supports provided in a person’s or family’s (“sponsor’s”)
home. The sponsor is evaluated, trained, supported and supervised by a provider agency that
follows the rules for licensing by DBHDS.

opic- Medication History
Therap's medication history form is the form used to create the electronic MAR. Sponsor's are required to enter the data in the med history form as well as uploading scripts or orders for medication verification. Sponsor's are responsible for the medical care and usually have the information about new medications, changed medications or discontinued medications before anyone else does. Assistance and supports should be provided by the QIDP, or the Medical Quality Assurance Coordinator as needed.
The videos uploaded will provide step by step instructions for how to enter a new medication, enter a medication change or discontinue a medication.
